relationship advice (post new situation)

"What should I do?"

Here are the general facts:
1) I'm in high school
2) I've been dating my girlfriend for over a year.
3) We do orchestra together every week.

These past few weeks have been rough for us. Every week I have to apologize about something I said but didn't mean. For example, I told her that I don't want to pick her up when we make out because I'm not strong enough. She saw that as she's too heavy. Another time I said a joke that came out wrong and made her think I'm calling her fat. And one time she asked me if my good friend was pretty and I said I guess and she flipped. Last night she went ballistic on me, and basically said that I have to fix our relationship, and she won't do anything to help. She also said that she's tired of being hurt by me, when I'm never trying to, and I'm painstakingly watching every word I say but it's never enough. I don't know what to do now. Am I the bad guy in the relationship? I've shown her for a year what she means to me but now she doesn't believe any of that and doesn't consider it either. I love her but I'm warn out. I'm afraid I won't even meet someone as awesome as her. What do I do? Do I break up with her?
